If you intend to use insurance, be sure to indicate your insurance information on the form. This helps us streamline and expedite the insurance verification process for you. For patients with insurances we do not accept, our office uses Reimbursify to simplify insurance claims for reimbursement.

Upon submission of your Prospective Patient Form, Dr. Aeva will review your information and determine if we are the right fit for your needs.

Once admitted into the practice you will receive an onboarding packet from our team. Included in your onboarding is a Clinical History Form used to tailor our approach to your specific needs. It gathers information about your mental health history and treatment goals. This is crucial in preparing for your initial evaluation.

At this step we also create a patient portal with a secure messaging feature to communicate directly with Dr. Aeva, so you always have direct access to discuss your care.

While completing your onboarding, our office will contact you to schedule your Initial Intake Evaluation where, together, you and Dr. Aeva will develop a personalized treatment plan that addresses your unique needs.
